Applications are available at all Columbus Recreation and Parks recreation centers, 1111 East Broad Street, Columbus, Ohio 43205 or by emailing volunteers@columbus.gov.  When requesting an application please specify your name, mailing address and number of applications needed.   Proof of Eligibility must be submitted with your application.  The four ways to prove eligibility are: Copy of current Medicaid / Care Source Card / Molina Card or a copy of current ADC Benefits.  A letter from the child's school stating he/she is on free lunch.  A copy of your most recent federal income tax return Scale based on HUD Guidelines, see chart below.

The above HUD Section 8 Guidelines will be valid through the Summer 2008 session.

You only need to meet one of the four methods to qualify.  PLAY does not pay for the School's Out Program. Any additional fee balances beyond the specified maximum amount will be the responsibility of the applicant.
